We focus on providing end of life care in a person-centred, sensitive manner – taking into consideration the wishes of our clients and their loved ones.

The UK is considered a world leader in palliative and end of life care provision and research and has influenced end of life care around the world.

How people are cared for at the end of life has risen to the top of the health care agenda in the UK.

Practitioners are bound by policy to provide quality palliative care to everyone who needs it.

Patients with palliative care needs are often admitted to hospital improperly when their condition deteriorates.

Yet evidence has shown that good access to 24 h community care is likely to reduce the number of emergency hospital admissions.

While most patients desire to die at home, the transition from hospital to community settings often lacks streamlined coordination of care.

Our assistance can ensure that adequate support is provided in the person's preferred care setting.

Our personalised Care and Support plans will be tailored to your loved one's specific needs as well as involving your family and friends, if appropriate. Every plan is different but can include the following areas:

  • Personal care and grooming
  • Managing pain
  • Medication for distressing symptoms
  • Companionship and reassurance
  • Continuity of care with carers you know
  • Continuity of carers
  • Implementation of personal wishes
