End of Life care is the treatment, care and support provided to people with a life-limiting illness, their family, and friends. Some refer to this as ‘supportive care’. This also includes planning for end of life.

What is end of life care?

This care plan entails the treatment, care and support for people who are nearing the end of their life. End of life care is designed for people who are thought to be in the last year of their life, however, the timeframe can be difficult to predict.

End of life care aims to:

  • Help your loved live comfortably as possible in the time they may have left.
  • Help your loved one manage the physical symptoms they are experiencing.
  • Provide emotional support to your loved one, family and friends. Having a trusted and friendly face around which you can confide in and discuss your concerns with can go a long way.

End of life care also involves, the team of people looking after your loved one, discussing their needs and wishes, taking into consideration what they want in the care provided.

How we can assist

Our dedicated care team provides a holistic care service working closely with healthcare professionals such as GP’s Manchester University NHS Foundation Trust (MFT), hospital discharge teams and pharmacists.

Our focus is on providing end of life care in a person-centred, sensitive manner – taking into consideration the wishes and needs of the clients family and friends.

Patients with palliative care needs are often admitted to hospital improperly when their condition worsens. Yet evidence suggests that good access to 24-hour community care is likely to reduce the number of hospital admissions. Our end of life care services can ensure that appropriate support is provided in the person’s preferred care setting.

Our personalised Care and Support plans will be tailored to your loved one’s specific needs as well as involving your family and friends, if appropriate.

Every plan is different but can include the following areas:

  • Personal care and grooming
  • Managing pain
  • Companionship and reassurance
  • Continuity of care with carers you know
  • Implementation of personal wishes
